If you are interested in enrolling your child in the SOARCE program through the Madison District Public Schools, you can choose one of the options below:

If you choose to download the application: bring the completed application to the Madison Schools Central Office located at 26524 John R., Madison Heights, MI 48071. For assistance or directions, please call 248-399-7800


The documents below are also needed when the application is submitted:

1. Two proofs of residency from the following list:

  • Valid Driver's License/Photo ID
  • Recent property tax statement
  • Current lease/rental agreement/mortgage documentation
  • Current utility bill
  • Homeowner's renter's insurance statement
2.  Child's original / certified birth certificate

3.  Special Services Needs / Please attach your students' I.E.P.

4.  Immunization records - submit copy of most current record
